Dr. Jeffrey LeSueur's Practice is dedicated to providing you with the highest quality, cost effective medical care. We accept all major credit cards, cash and check.

Please remember that it is your responsibility to be familiar with your insurance plan and your benefits. Please see our full financial policy located in our new patient registration packet, which you can find in multiple locations on our website. Please call our office with any billing questions or concerns.

Dr. LeSueur accepts most major insurance companies. If you do not see your insurance listed here, please contact your insurance company to find out if we are a participating provider. Please remember that you are responsible for making sure that our office receives a referral if it is required by your insurance.

  • Dr. LeSueur is NOT contracted with commercial United Healthcare plans. We can still see you if you have out-of-network benefits. Please contact your insurance to understand your benefits and to find out whether we're covered.

Additional Fees: 

 

There is a $35 dollar no-show fee if you miss your appointment without notifying the office in advance. Your appointment time is reserved especially for you. If you miss your appointment without notifying the office, we are unable to provide care to someone else who also needs our services.
